How do I configure a login for a site which requires: 1) username 2) pin and 3) password?

Hope your question is about the Identity Safe feature, to save logins for the web page which requires 3 user credentials for authentication.

Identity Safe feature supports only webpages with 2 login credentials. So as of now you will not be able to save the logins for the webpage which has more that 2 login credentials.
