Here's what I'm seeing when right clicking a file under XP SP3. Is there any particular reason to have "Norton File Insight" is listed twice: once in the sub menu and once at the bottom? Could the extra one at the bottom be removed?

Morac,
One is Norton Insight network scan. It takes information from the network to perform the scan on the particular file. This scan will query the Quorom database in Norton and subject it to a scan. If you perform the Norton Insight Network Scan, here is how the results will appear.
While the other option Norton File Insight will provide information about the trustworthiness of the file in contention. Based on the trustworthiness, you can choose to scan it or allow it to exist.
For the same file (I took NRT as an example), Norton Insight Network Scan shows if it is infected or clean. While the Norton File Insight shows if it is a trusted file.

mythbuster,
I believe that Morac was asking why Norton File Insight appears at two places on the context menu. It appears under Norton Internet Security, and then appears again as the next to last item in the context menu.

And mythbuster answered the question: one is for the online scan, and one is for the File Insight "info-window".

Brubaker is 100% correct. While mythbusters post was very informative and the graphics were excellent, it did nothing to answer either of the OP's 2 questions. Norton File Insight is listed twice in the context menu, since clicking on either of those options produces the same result, this would appear to be redundant. So the OP's questions remain unanswered. 1. Why is it listed twice? 2. Can the one at the bottom be removed?

Turbo
How is the scanning of the file and the file database information display the same thing? There are two different processes that can be invoked from the menu.

The OP and Turbo are correct. The same entry appears twice in the context menu.
