Anytime you get a screen like that, that asks for information that the site either already has, or shouldn't need to know (SSN and DL numbers are not necessary to access your online account) you should immediately suspect something criminal is going on.  First, make sure that you are entering the correct URL for Bank of America.  Make sure the SSL cerificate checks out (click the padlock in the address bar or status bar of your browser).

Download, update and run a full scan with the FREE version of Malwarebytes' (Blue Download button) to see if it detects anything on your PC that might be redirecting your visits from legitimate bank sites to fake phishing sites.
