unable to submit WebPulse Site Review Request

despite filling in every form field on the Norton "WebPulse Site Review Request" form I cannot submit this as the button just remains grey and cannot be clicked.

This means music.amazon.com remains in the shopping category rather than entertainment / music and subsequently my kid can't access given I want to restrict access to shopping sites but allow them to listen to music.

I'm using Google Chrome on a Mac.

Anyone else had the same problem?

Please advise.

We have tested the category for "music.amazon.com" and it is categorized as "Entertainment".
