Jotform form getting flagged as suspicious web page

I am having a problem with a form I have hosted at popular form provider Jotform being flagged as suspicious by the Norton Security Toolbar.

The form is at : https:// eu . jotform . com/chapter_one/131809210381

The domain https : //eu . jotform . com is whitelisted by Norton and I do not believe that I have any suspicious looking text or info on the form.  Can anyone advise how I can get the form cleared as far as Norton are concerned because it will cause alarm to my customers.
