Notify a Moderator redirect?

Occasionally I am redirected after clicking on Notify a Moderator to a page asking to confirm I want to notify a mod. When I click on the confirmation  button I am just returned to the original thread page. I have to click Notify a Moderator again to be able to make my report.

I missed the screenshot, but will try when I see it again.

That's what I have been seeing. But it is  not every time. I have not noticed any common cause for this coming up. Most times I do not see this.

That's an old experience. I will confirm with the dev if it gets triggered in any specific case or it's a bug that needs to be fixed. Thanks for reporting.

Just noting that I am still occasionally seeing this issue.
