Windows defender virus

So when i downloaded a suspicious file windows defender blocks it and asks me to run it or not. So, during that pop up could that virus start attacking my pc or will windows block it until i tell it to run or will it already be running and screwing with my pc?

When SmartScreen appears it usually says the app you want to install is unrecognized. The filter then leaves you with only one button to push: Don't run.

By showing only one option, Microsoft hopes to prevent the majority of users from running untrusted apps, because many won't bother to look beyond that single button.

If you still want to take the risk and proceed, click the More info link at the end of the warning paragraph.

When you click that, you then see a window like this one with a new option: Run anyway. Click that, and you're all set. The program will start installing as normal.

If it finds a match, SmartScreen will warn you that the download has been blocked for your safety. SmartScreen also checks the files that you download against a list of files that are well known and downloaded by many people who use Internet Explorer. If the file that you're downloading isn't on that list, SmartScreen will warn you.

Yes basically, but instead of it looking modern like that it was the old windows 7 prompt. So, would the virus already run or would windows block it?

You're on Windows 10?

Why Windows 7 looking prompt?  
Wonder if your W7 prompt is legit?
I've only seen modern look prompt...long time. 

In theory "run" is waiting user action.  W7 looking prompt was "download was blocked" or "warn you". 

SmartScreen will warn you that the download has been blocked for your safety.

No sorry i just need to know if the virus would run even before windows warns me about opening it.

just the other day I got

I saw no indication that the TaskExplorer ran.  No indication is not a guarantee, IMO. 
and I also got

I did not see your W7 looking prompt event....nor activity leading up to your W7 looking prompt event.
