Upon further analysing the backend data, we observe that child's device was deleted and hence Norton Family portal shows the message "child is currently not being monitored and add a device".
Subsequently we see that another iPad has been added to the child recently, Please try and let us know if you are facing the issue again.
Note: When Time Supervision curfew or Instant Lock is enforced on an iOS device, custom apps on the device disappear and then reappear once the curfew ends. Please check and readjust curfew settings as required.
