Does real time defrag run all the time in the background? Where can I go to find out more about it?

Yes and no.
My understanding of real time defrag is that it runs in the background whenever it is needed.
At a certain point, the disk is defragmented so it would not then keep running or defragmenting (although it would ideally be available should a file become fragmented).

If this is done correctly, there would be no resource hit on the computer while real time defrag was running; it would be truly transparent, as described in the article above.
You should avoid real time defrag if you are using imaging apps.
