Multiple logins saved for same site

Norton Password Vault has taken to treating each time I login to a site as a new event that requires saving a new password.  Now I have some sites that have multiple entries in the Password Vault which are then reported as me using the same password for different sites.  While I can delete the duplicates, how can I stop this from happening in the future?  This is on Windows 10 using Firefox.
