Can't Get Rid of Trojan.Gen.2

I tried to follow the instructions in the post below because I have a very similar problem.  Randomly, my computer would fail to start and automatically begin a repair which takes about 30 minutes to complete.  Sometimes the repair fails and then automatically shuts down.  Afterwards, my computer would start like normal with no issues.  I downloaded OTL and scanned my computer with the 60 days setting.  The notepad files from the scan are attached.

The file that repeatedly shows up is called 80000004.$ and it's identified as Trojan.Gen.2.  The auto detect has found over 35 of these same files.

 

On occasion, I would also get "kwrd.dll" (Trojan.Gen.2) and "consrv.dll" (Backdoor.Trojan).

You have or had, max++ aka Zeroaccess infection.

Is there anything else I need to do?  I ran the scan, but have no idea what to do with the notepad documents that I received from the "Run Scan" process.  Is there anything I need to modify before I copy and paste the txt from the notepad docs into the "Custom Scans/Fixes" box so that I can perform a "Run Fix" with OTL?

You have no custom script to run, the other thread had an extra infection to mop up afterwards and is for that machine only.

 

OTL does not remove zeroaccess and is an advanced tool.  There was a step before using OTL that removed zeroaccess on that thread.
