Since the original thread on FF 4.0 toolbar is marked solved but some of us have a few problems after the Hot Fix I opened this new thread.
After installing the Hot Fix I have a few problems. I have tested all my entries in Identity Safe and have 7 sites that are broken. These sites worked in FF 3.6.16 and IE-9 but after applying the Hot Fix either the Login or Password fields are blank. The other 40 sites now work fine in FF 4.0, FF 3.6.16 and IE9. I reinstalled NIS 2011 and the problem cleared but when I reinstalled the Hot Fix they are broken again, so it points to a bug in the Hot Fix.
Have you tried deleting any of those problematic logins in Manage Logins and then manually logging into the site and letting Norton recreate the login entry?

Bank of America can be a real pain. First make sure that the landing page is not saving and remembering your Username from session to session, as it will offer to do when you first log in. This can confuse things when Norton tries to enter your Username and the form is already filled in by BofA.
If you still experience problems, instead of logging in from the landing page click the "Sign in" link at the upper right corner of the page and then click "Sign in" in the Online Banking section of the following page. Sometimes this works.

I don't let my sites remember my login so it all comes for Identity Safe. The Login is OK but the Passcode on the following page is blank. It works if I don't install the Hot Fix. Also there is no Login on the right of the screen, just on the left side.

Your discription does not match BOA site. I did find on the main page "Online Banking" with a link to a diferent Login page, Login gets populated but when it takes me to the Passcode page it still fails.
As far as I'm concerned, Bank of America has always acted like that. And I agree with you on that the alternative provided by SendOfJive brings you to the the same spot.
One of the reasons this may be as such with BOA is possibly the step between user ID and PW that the bank requires you to confirm as correct before entering your Password. Will that be your case?
One other point: I experience the same as you describe, but this was already the situation BEFORE I upgraded to FF4. So, I do not see it as an issue related to the Symantec's patch.
I have other situations, without the possible "complications" of BOA, were either the User ID or both the User ID and PW will be missing. One such example is this:

Now that they have a two-stage log in with the visual "SiteKey" many applications (eg Quicken) do have problems.
Have you talked with B0A about it -- I know they gave my wife an alternative route to log in by when she had trouble getting in even when manually entering the data some time back.

So, I understand your situation is different from mine, as I never had that last field automatically filled. But let me ask you, how do you think that the site key fulfils any useful purpose, if the PW field is automatically completed in whatever case?

This one has a plain vanilla login format and still, it only fills the PW field and leaves blank the ID field. As someone suggested, I deleted and logged in manually, accepted the request from the Identity Safe, logged out, tried to login again and... Zippo! Same exact situation.

I use Acronis but as I said I don't use FF or Identity Safe functions in my Norton product. I only posted the image in case it helped since people seemed to have been seeing different screens.
