I do not understand. Quarantine has an item, hhzv30.exe (WS.Reputation.1) detected
by Download Insight, severity=Medium, from Nov. 12, 2011. "No Action Required".
So what do I do with it now? How do I remove it from the computer? How do I
send this information or file to Symantec to check it? The Security History
window does not give this/these options.
NIS 2011 v 18.6.0.29 fully updated, Windows XP sp3.

You must be logged on as an administrator. When you open Norton History to Quarantine, you will see a button to clear all entries. This will remove the quanantined items from history and from the computer.

The helpfile only says that Clear Entries removes the listing from the quarantine report but does not say that it also would remove the item from the computer. How do I remove the item from the computer. I want more than just a listed entry cleared - I want to remove the actual item from the computer.

Removing an item from the Quarantine
You can use the Remove this file option to remove the security risk and places it in the Security History Quarantine. Some programs may rely on the security risk item that you quarantine to function. In this case, you can restore the security risk to regain the functionality of a program that requires the risk program to run.
Note:
The Remove this file option is only available for the security risks that are manually quarantined.
To remove an item from the Quarantine
In the Security History window, in the Quarantine view, select the item that you want to remove.
Click Options.
In the Threat Detected window, click Remove this file.
This option is available for the security risks that are manually quarantined.
In the Security Risk Removal window, click Remove.
Click Finish.
To submit a File in quarantine click Options at the right corner of the quarantine window.

Items in Quarantine have already been removed from your computer and are being kept as encrypted, inactive files in case you should need to restore them. Clearing the Quarantine entries removes these archived copies, and nothing of the threat will remain on your computer. That's all you have to do.

Thanks, this means I can keep the item "encrypted" in Quarantine, or Remove it from the list, and I will still be safe. The help file did not make this clear.
To help clarify my question, I did not manually put the item into Quarantine. NIS did this automatically just after the download of the item. (hh...30... something... zip, downloaded a few weeks ago from Softpedia); but right now while I'm on a different machine, in case anyone is interested.

That is correct. Quarantine is designed to allow you to restore the malware in cases where its removal may have crippled your system or programs. Since this was a downloaded file that was quarantined based on reputation, you should be safe in clearing it (I assume your system is running well and that you do not need the file).
