Identity info not filling correctly

This is happening at

The form is asking for Phone#, but the autofill is placing my username for the website/forum into the phone field.

Even if I change the info in the field and try to continune to next form/page it defaults back to placing my username into the phone field and wont let me continue.... very frustrating

I have checked the Identity Card and even created a new one, but still same problem.

I have also deleted all history in Firefox as well as cookies.

Still having this problem

 

Any help?
