medium severity find in "recent history"

On starting my pc & checking "recent history" one item keeps being found & blocked.It is of medium severity and marked with a red dot.

  Clicking on more details,this appears:  ACTOR- c:\windows\explorer.exe

                                                          ACTION- send terminate message to window

                                                          REACTION- no actions available for this item.

Now I know this is being blocked,but can anyone advise what this is? A full scan reveales nothing except tracking cookies.
