This list of changes for the patch indicates that the issue with being prompted to create an Identity Card even though you've selected not to be asked again has been corrected:

- Corrected an issue that could produce a Blue Screen under certain conditions.
- Fixed issue relating to handle leak in ccSvcHst.exe.
This issue doesn't appear to have been fixed. If you select the "Don't ask me again" check box, you won't be asked again for the duration of that session. However, if you log off and log back on again, you will again get prompted to create an Identity Card when opening a web page that contains a form to complete.
