I found another mis-behaving log-in page. Go to http://www.golfgalaxy.com and click the "sign in" link at the upper right corner of the page. This brings up a log-in page with two log-in options: On the left you can log-in using e-mail address and password (2 fields) or on the right you can log-in using your Advantage Member Club Number and PIN (2 additional fields). I filled in the e-mail and password fields and click the "sign-in" link below, Identity Safe pops up the offer to save the log-in, I save it and all is well. Later when I return to Golf Galaxy and try to log-in, Identity Safe auto fills in the member number field on the right side of the page with my e-mail address and the password field on the left. This is the wrong field for the e-mail address and of course the log-in fails.
Any ideas on this one?

Late breaking news: Found another one that appears to be a similar problem. Go to http://www.shopping.hp.com and click the sign-in link. There are three fields on this page, on the left a field for e-mail address to subscribe to news letters. In the main log-in block there is an e-mail address field and a password field. As above I filled in the log-in e-mail address and password and the log-in was saved as expected. Later returning to the log-in page, Identity Safe auto fills the wrong e-mail address field with my e-mail address and puts my password in the right place.
