Startup Repair...AGAIN

Hello. I had an issue with Startup repair last year. Last year it had failed to repair any problems with the computer and I was forced to do a system restore. Now it's happened again. This time, it didn't look like I had to do a system restore as it had appeared to repair it itself and I was able to return to the computer. However, NISv20 was inoperable. I could not access it at all. So, I uninstalled it and reinstalled NIS 2012. I'm in the process of upgrading to v20 again right now. Is there any reason why any of this had happened and how I may be able to prevent it in the future? Here what Reliability Monitor had to say...
