Trojan found during full system scan

This month when I did a full system scan, it found a trojan. This is the first time this has happened. Usually it will catch a trojan or virus when my browser encounters it. It resolved the threat (I guess that means it removed the trojan), but I have some concerns.

1. Before I run a full system scan, I always open my browser (IE 7), go into the Tools menu, and delete all cookies, temp files and history. I did this before running this scan, too. Yet it found the trojan in a temp file????

2. How is it that the file got into my system when my virus scan is always running? How did it get by the anti-virus software?

3. It could have been in my system for as long as a month, when I ran the last scan. Is there any way to know whether and what information it got from my computer while it was in the system?

4. It didn't name the trojan, and the ID number given doesn't come up when I search the virus DB on this website. It does give a temp filename. Can anyone use the information from the log (below) to help me identfy what it was?
