Receiving "Failed to save login" on Firefox & IE web browser

Hello everyone,

I have been experiencing an issue for the past two days now.  In Identity Safe, my vault is currently open.  Whenever I visit a website and login with my username/password, I'm continually receiving the message, "Login saved to vault, failed to save login."  I also went to the Identity Safe login page to manually enter website url with the username/password and it says "an existing login already been created for this webpage, please create another username/password".  This is so frustrating.  I'm not sure what the problem is.  I've had two representatives already remotely access my computer to attempt to fix the problem with no solution.  I had a previous version of Norton that I also used Identity Safe for, but I forgot the password.  After numerous failed attempts, I deleted the vault and created a new one with a new password.  The funny thing is I have installed Identity Safe for mobile (Iphone) and it is showing my three logins that I created on yesterday, but nothing on my laptop.

Any help is appreciated.  -Windows OS, Vista Home Premium, Browsers affected are IE9 and Mozilla Firefox v35.0.
