Something altering Google Search results

note: also posted this on google's help forums.  I noticed another threads there complaining of the same issue

 

This problem only occurs when I am not logged into my google account.
The name on the link and description match my search, but the actual URL does not.  I have cleared my cache, deleted all cookies, restarted, and done top to bottom system scans with Symantec Antivirus (virus definitions up to date), without anything being found [I have also installed and run lavasoft's ad-aware since, also returning no suspisious entities]

top 4 results while logged out of google:
